Cerebral uptake of Tc-99m methylene diphosphonate (MDP) due to cerebral infarction in elderly patients has been reported previously. Our patient was an 18-year-old female with increased MDP activity in the right semihemisphere of the brain. Subsequent cerebral angiography revealed occlusion of the right internal carotid artery, consistent with a diagnosis of Moyamoya disease.
